Once saved is NOT forever saved. There are several warmings of the possibility to fall from the grace received and there are examples of those who once saved but went into apostasy.
Warning On Possibility Of Apostasy:
1 Corinthians 10:12 says, "Wherefore let him that thinketh he standeth take heed lest he fall."
Hebrews 2:1 says, "Therefore we ought to give the more earnest heed to the things which we have heard, lest at any time we should let them slip."
2 Thessalonians 2:3 says, "Let no man deceive you by any means: for that day shall not come, except there come a falling away first, and that man of sin be revealed, the son of perdition;"
1 Timothy 4:1 says, "Now the Spirit speaketh expressly, that in the latter times some shall depart from the faith, giving heed to seducing spirits, and doctrines of devils"
2 Timothy 4:3 says, "For the time will come when they will not endure sound doctrine; but after their own lusts shall they heap to themselves teachers, having itching ears; 4:4 And they shall turn away their ears from the truth, and shall be turned unto fables."
Acts 20:29 days, "For I know this, that after my departing shall grievous wolves enter in among you, not sparing the flock. 20:30 Also of your own selves shall men arise, speaking perverse things, to draw away disciples after them. 20:31 Therefore watch, and remember, that by the space of three years I ceased not to warn every one night and day with tears."
Hebrews 6:6 says, "If they shall fall away, to renew them again unto repentance; seeing they crucify to themselves the Son of God afresh, and put him to an open shame."

Biblical Examples of Those who Fell:
1. Ananias and his wife- Acts 5:1-13
2. Hymenaeus and Alexander -1 Timothy 1:20
3. Judas Iscariot fell - Acts 1:25
4. Some people who turned to be antichrists in the first centuries fell away from the faith - 1 John 2:18-19
Conclusion: No one is saved once and forever. It's only those who are faithful till death shall be saved eternally -Revelation 2:10
